REAL Care Team Leader

JOB SUMMARY
This full time, at will, position is responsible for providing day-to-day support to a team of specialized Options Counselors and Care Managers. This leader provides support and guidance, motivating the team to meet the group’s responsibilities with the highest quality of service. This position supports the agency’s caregiver support Title III-E programming and the agency’s fee-for-service managed care options counseling program through enlivIN.

ESSENTIAL DUTIES include the following. Other duties may be assigned and/or requested from time to time in fulfilment of the agency’s mission.
1. Provides leadership to the team and works with members to build unity and cohesiveness
2. Assists with the hiring process to fill vacant positions in team
3. Contributes to the training process for new staff members
4. Offers all team members training, coaching, and re-training as appropriate
5. Inspires person-centered care philosophy
6. Assures the agency Personnel Policies and Code of Ethics are followed
7. Ensures program timelines and guidelines are met
8. Ensures team responsibilities are met, performing staff duties when temporary shortages of staff occur
9. Serves as a liaison between staff, A2AA administration, State staff, individuals, families, and other community stakeholders, providing outstanding customer service
10. Manages administrative responsibilities for at least two team members, including but not limited to, processing time sheets, time off requests, expense sheets, staff evaluations, benchmarks, and reports
11. Monitors quality outcomes of the team, communicates and assists in building team understanding and compliance with expectations for outcomes
12. Communicates information to the team, passing on information in a timely manner while retaining the integrity of the message
13. Reviews and approves Service Plans
14. Serves individuals in a manner that is empowering, patient, compassionate, and person-centered
15. Attends, participates, and leads meetings and trainings
16. Maintains the team’s workflow

RESPONSIBILITIES
This position makes a substantial contribution to the mission of REAL Services, Inc. due to the fact that this person provides the leadership to build and maintain a solid and well-functioning team as the Agency’s representatives to the community. The attitude and culture of a team is directly influenced by the qualities of this individual. In addition, the quality of care delivered to clients is dependent on whether this individual has helped to cultivate and inspire an effective, organized, and coordinated team.

EDUCATION AND/OR EXPERIENCE REQUIREMENTS
Bachelor’s degree (BA or BS) from an accredited four-year college or university in any field.
– Obtain and maintain ongoing Care Management certification.

KNOWLEDGE. SKILLS. AND ABILITIES REQUIRED

3. Ability to organize workflow and manage workload, applying sound judgement
4. Excellent communication skills, both in writing and orally, with good interviewing skills
5. Ability and attitude to work productively, independently, cooperatively, and relate to the needs of individuals, colleagues, the agency, and community partners
6. Ability to follow instructions and procedures, to interpret the information and to complete assigned tasks in an accurate, concise, and detailed manner
7. Knowledge of in-home and community based services, funding sources, eligibility, community resources, and information available through REAL Services and in the community
8. Ability to assess specific needs both in-person and on the phone
9. Ability to develop rapport quickly, and to empathize and understand individuals’ behaviors
10. Ability to use state mandated computer programs, as well as basic knowledge of Excel, Access, and Word computer programs, as relevant to successfully execute the job

DIFFICULTY OF WORK
Guidelines are generally clear and complete, with some judgment required in implementing them when unusual circumstances arise. The work demands sound judgment be exercised in selecting the most tactful approach in communicating with team members and with individuals served. All documentation must be accurate and timely. The work is repetitive in nature inasmuch as the same tools, forms, and procedures are utilized routinely, but does involve variables and considerations when dealing with diverse situations.

The physical demands and work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is usually seated, standing, climbing stairs, descending stairs, or walking at will in a typical office setting. Very limited physical effort required. Regular and recurring travel to assessments, meetings, trainings, and presentations will be performed. A valid driver’s license and a dependable vehicle is required.

CONDITIONS OF EMPLOYMENT
1. Successful candidate must submit to a pre-employment drug screening and a limited criminal history check
2. Willingness and ability to learn required software
3. Proof of educational credentials and/or licensing is required at time of employment
4. This position has a 6 month probationary period.
