They provide Options Counseling to identify the individual’s strengths, values, and preferences.

They identify resources available to individuals and their circle of support (formal and informal supports including community organizations), and
screen individuals for potential eligibility for funding sources and programs.

As part of the whole person approach, we determine eligibility for on-going home and community-based services, and assist in accessing home and community-based services that would meet identified needs.
