Adult Guardianship Services

Our guardianship program recruits, trains, and supervises specially trained community volunteers to provide support and advocacy to those who are unable to care for themselves.

This program is part of a statewide network, Volunteer Advocates for Seniors and Incapacitated Adults (VASIA.) The program enhances the quality of life of those it serves by making decisions on their behalf that reflect, to the maximum degree possible, their own life choices and preferences. Guardianship is a legal process in which a court appoints a guardian for an adult who cannot manage their affairs. The guardian assists with essential decisions, such as protecting the person's finances and making critical health-related choices. The individual’s history, needs, and preferences are carefully considered throughout this process. The guardian also provides encouragement and reassurance, helping to ensure the person’s daily life is as positive and supportive as possible.

To be eligible for the Adult Guardianship Services Program, an individual must:

  • Be at least 55 years or older
  • Have been determined to be unable to make medical and financial decisions for themselves
  • Have no family or friends able, willing, or suitable to serve as a guardian
  • Live in St. Joseph, Elkhart, Marshall or Kosciusko Counties in Indiana

Referrals to the Guardianship Program are reviewed monthly by the Guardianship Advisory Council. A referral document, a physician’s statement, and an individual’s most recent physical and medical history must be submitted by the Thursday before the 3rd Wednesday of the month in order for that individual’s case to be reviewed. After reviewing a referral, REAL Services petitions the court to become the individual’s guardian.

Through the Guardian Partner Program, REAL Services invites Attorneys, Fiduciary Entities and other professional providers the opportunity to partner with us in the care of seniors unable to make their own financial and medical decisions.

This cooperative program is generally a private pay client.  A client’s history, needs, and wishes are used to inform us in decision-making, and offer the encouragement and reassurance necessary to help make every day as positive as possible.

In order to be eligible for the Guardian Partner Program, an individual must:

  • Be at least 60 years old
  • Be mentally incapacitated and unable to make medical and financial decisions for themselves
  • Have no family or friends able, willing, or suitable to serve as a guardian
  • Live in St. Joseph, Elkhart, Marshall or Kosciusko counties in Indiana
