Full Job Description
This full-time position is responsible for ensuring individuals waiting for care receive quality attention and their experience is coordinated with other Area Agency initiatives. This position requires that the Needs Analyst realize the complexity of issues affecting those awaiting assistance, including caregiver needs and the critical importance of accurate data. This position will be paid from SSBG, Title III-B, CHOICE, and Medicaid Waiver.
ESSENTIAL DUTIES include the following:
- Maintain contact with individuals on the waiting list. Close contact is maintained with individuals at the top of the waitlist.
- Manage and track MDS referrals.
- Complete Waitlist and MFP targeting.
- Provide comprehensive options counseling.
- Update waiting list information with appropriate demographics, and individual needs.
- Complete required documentation and reports in a concise manner.
- Compare data to insure the integrity of information.
- Update and maintain CaMSS records.
- Ensure terminated individuals are processed appropriately.
- Track the disposition of terminated individuals.
- Follow the current targeting plan, ensuring monthly quotas and timeframes are met.
- Ensure that wait list integrity is maintained.
- Speak knowledgably on the needs of the un-served.
- Attend staff meetings and training sessions.
- Consult with supervisor and alert supervisor when consultation is required.
- May assist with other referrals and care management tasks.
- Other duties as assigned.
Those individuals who are waiting for assistance may have complex situations. The right candidate will realize the importance of ensuring that those who need assistance receive whatever help we can provide, while ensuring that only those in need of help remain on the wait list. In order to adequately advocate for the needs of those who wait assistance we must be certain of the credibility and accuracy of our information. It is the responsibility of this individual to ensure accuracy of information while also ensuring a compassionate and humane approach to serving older adults and those with physical disabilities.
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
Bachelor’s degree (BA or BS) from an accredited four-year college or university in social work, psychology, gerontology, counseling; or a license as a registered nurse with one year of experience in human services; or a bachelor’s degree in any other field with a minimum of two years, full-time direct services experience with the elderly or disabled persons. This experience must include assessment, plan of care development, implementation, and monitoring. Or applicant may have a master’s degree in a related field to substitute for the required experience. (Division of Aging Application for 1915 ( c) HCBS Waiver: IN0201.R06.00- July 01, 2018)
Maintain ongoing Care Management certification.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Ability to assess a client’s specific needs and plan solutions; to understand and relate to behavior of individuals; to support the mission of the Agency and meet the requirements of good case management.
- Ability to perform with a team concept and follow program requirements.
- Ability to communicate orally and in writing; to convey decisions to a client or family in a tactful and diplomatic manner.
- Understands Medicare, Medicaid, private insurance, and other payment options.
- Good interviewing, organizing and assessment skills; knowledge of the geographical area.
- A specialized knowledge of principles and practices developed through relevant experience after a baccalaureate education.
- Ability to interpret information and certify ongoing eligibility for programs and services being authorized on the Service Plans.
- Ability to use CaMSS and other computer programs, as relevant to successful completion of the job.
DIFFICULTY OF WORK
The position demands judgment is exercised in selecting the most appropriate method to convey information to an individual or the client’s family.
The work is repetitive in nature inasmuch as the same forms and procedures are utilized over and over, but does involve variables and considerations when dealing with diverse situations.

While performing the duties of this job, the employee is usually seated, standing, or walking at will or meeting with clients/applicants in their home. Limited physical effort required. Travel is periodically required.

CONDITIONS OF EMPLOYMENT
- Successful candidate must submit to a pre-employment drug screening, a limited criminal history check, Drivers background check, and Sex offenders registry check.
- Proof of educational credentials and/or licensing is required at time of employment.
- This position has a 6 month probationary period.

Pay: $36,000.00 – $40,000.00 per year
